Fertility Assessment & Counseling

Fertility Assessment & Counseling in Delhi

Very often we get to see women walking alone into the clinic of a Fertility specialist to get herself checked as she is unable to conceive after many years of marriage. In 40% cases of infertility, it is the female factor, and for another 40% it is the male factor and 15% cases are due to combination of both male & female factors and for the remaining 5% the cause is unexplainable.

When should a fertility assessment be done?

  • If you have been trying to get pregnant for longer than a year.
  • If you are over 35 years old, as women's reproductive capacity declines with age.
  • PCOS and a history of recurrent miscarriages.

What is included in Fertility Assessment?

  • Male fertility assessment

Semen Analysis- It evaluates factors like sperm count, motility (movement), and shape to determine the quantity and quality of a man's semen and sperm.

According to WHO, male factor infertility is an alteration in one or more of the following parameters in one of the two samples collected 4 weeks apart:

  • Sperm count – 15 million/ ml of semen
  • Motility - 40% sperms should be motile
  • Morphology (shape) – 4% normal forms

Female fertility assessment

  • Ultrasound on Day 2 of period
  • Anti-MullerianHormone(AMH) Test
  • Co relation with age
  • Body Mass Index Assessment
  • Hysterosalpingogram or HSG
  • Hysteroscopy/Laparoscopy
  • Review with Fertility Specialist

Semen Analysis:

It measures the quantity and quality of a man’s semen and sperm – assessing characteristics such as sperm count, motility (movement) and shape.

Sperm cryopreservation (freezing) is recommended to preserve fertility in men staying away from their family or undergoing cancer therapy.

Anti-Mullerian Hormone (AMH)

Levels are assessed by a simple blood test that can be done on any day of the cycle and helps to know the capacity of a woman to produce eggs at any given point of time. The lesser the no of eggs in the ovary, lower will be the AMH level.

AMH Levels

Less than 1.0 ng/ml

Low ovarian reserves

1.0 -3.0 ng/ml

More than 3.0ng/ml

Body Mass Index Assessment :

Obesity affects fertility in both men and women, which decreases the likelihood of getting pregnant. The best BMI range for conception is between 18.5 to 24.9, which also increases the likelihood of a healthy pregnancy and child.

Ultrasound on Day 2 of period:

On the second day of the period, a transvaginal ultrasound is performed to count the number of antral follicles.

HSG, or a hysterosalpingogram:

HSG is suggested in young patients (less than 30 years old) with good ovarian reserve and a normal husband's semen analysis report to determine whether the tubes are open or blocked. It is an X-ray test done by the fertility specialist by injecting a harmless dye into the womb, which should flow into the fallopian tubes. An X-ray with the dye visible reveals whether the tubes are blocked or open. The next step would be a laparoscopy and hysteroscopy if the tubes are blocked .

Laparoscopy/hysteroscopy is the gold standard to assess the uterine cavity, tube-ovarian relationship and patency of the fallopian tubes

Review with a Fertility Specialist: Your fertility specialist will explain the reproductive potential after all the investigations are complete. If all of your test results are normal, the fertility doctor can recommend additional.

A fertility assessment gives us a clear picture of the reproductive ability of the couple along with reassuring them that medical technology can help them become parents if there are any factors causing infertility.

The success rate of IUI is approximately 20%.

What is the difference between stimulated IVF and Natural IVF?

Stimulated IVF- A woman takes medicines/injections for 9 to 12 days to stimulate both of her ovaries to generate and mature multiple egg follicles. During the stimulation phase, a physician will do ultrasounds to track the follicles growth until they reach a specified size (18-22mm). Natural IVF- In this cycle, instead of stimulating your ovaries to produce multiple eggs, the Natural Cycle IVF Process is meant to select that one egg that your body produces naturally.

What is the advantage of natural IVF cycle?

Multiple pregnancy risk is reduced. Reduces the high costs associated with stimulation medicines. Reduces the risk of drug-related adverse effects.

What is Controlled ovarian hyper stimulation (COH)?

Controlled ovarian hyper stimulation is a procedure in which the patient’s ovaries are stimulated for 10-12 days with hormone injections (gonadotrophins) in order to obtain the highest number of mature oocytes (eggs). The response to these injections is tracked by ultrasound, and after the cohort of follicles reaches 18-22mm (minimum 4-5), a trigger injection is administered, followed by ovum pickup (egg collection) 34-36 hours later.

What are the risks associated with controlled ovarian hyperstimulation?

Ovaries of some females might respond excessively to the fertility drugs. OHSS(Ovarian Hyperstimulation Syndrome) occurs in a minority of females who overrespond, symptoms of which include, vomiting, nausea, severe discomfort, abdominal distension and dehydration. Note: The patient should immediately notify the clinician in case of such symptoms
